IIR - Crew Pass Out in BORF

- 0750 - 1000 hrs: Toolbox meeting ended, and Tony started working at Train A to remove 4 units
of temporary flushing spools with 5 others co-workers
- 1000 - 1015 hrs: 15 minutes Tea Break
- 1015 – 1200 hrs: Continue with work activities at Train A
- 1200 – 1300 hrs: Lunch Break
- 1300 – 1400 hrs: Reported leakage at Train B, Tony and 5 others were asked to re-secure the
80mm bolts & nuts with impact wrench. The job was completed at approx.
1400hrs
- 1400 – 1500 hrs: The team remain standby at Train B to ensure no further leakage
- 1500 – 1515 hrs: 15 minutes Tea Break
- 1515 – 1615 hrs: The team remain standby at Train B, when Tinggi Angil (Eye witness)
noticed that Tony was passing out at approx 1600hrs. SHO on-site & Con.
Supt. were notified. Bintulu PMTs were reported that no bodily injury was
observed
- 1615 – 1700 hrs: Tony was advised to be sent to Bintulu Medical Centre (BMC) for treatment,
accompanied by SHO on-site & Graduate Engineer. SHO informed that Tony
has re-gained consciousness when inside the company vehicle, approx 1620hrs.
- 1700 – 1900 hrs: Tony arrived at BMC approx 1700hrs. Medical Doctor has carried out ECG
dialysis (Normal), checked on glucose level (Normal) and vital sign (Normal);
and concluded that Tony can be discharged , with no further treatment/check-up
required, after 500ml normal saline drip.
INCIDENT INVESTIGATION
- During the interview session, Tony mentioned that he was not having his breakfast nor lunch, and
only drank water since he started working in the morning
- He has completed his work task and remained on standby at the worksite together with other co-
workers. He felt dizzy, and managed to sit onto the scaffold platform prior to passing-out. The
process was witness by one of his co-worker (Tinggi Angil – rigger)
- SHO on-site reported that no bodily injury was observed, and the Medical Doctor at BMC has
confirmed on the observation
- The doctor has carried out ECG Dialysis, checked the glucose level and vital signs (including
pulse, body temperature, blood pressure and diet). All tests results were normal, and the doctor
has concluded that Tony was having too little energy source for his body
- Tony was discharged after having 500ml of normal saline drip, and required no further treatment

RECOMMENDATION
- Doctor recommend Tony to maintain a healthy diet habit (i.e. having less food, or smaller portion
of food for diet purposes) and avoid not having breakfast & lunch
- PMTs’ reminded Tony to stand-by some light food (i.e. biscuit, sandwich etc)
- PMT also requested Tony to assist at Fabrication Yard from 02.11.2013 to 03.11.2013, for
monitoring purposes
- PMT concluded this incident as a Non-Work Related Incident (Health issue). However, Lesson
Learnt will be cascaded to all crews abroad
E. INCIDENT NOTIFICATION
- On-site SHO and Construction Supt was informed of the incident @ approx 1600hrs
- PMT (BTU) was notified @ 1615 and advised on-site personnel to sent Tony to BMC immediately
- Project Manager were notified via telephone, approx 1700hrs
- DE-F-36 (Accident Notification) was filled and notified via email, approx 2000hrs
