AFFIDAVIT OF TWO DISINTERESTED PERSON

WE, JOVITA V. SICAT, 89 years old, widow and a resident of Poblacion West,
Science City of Muñoz, Nueva Ecija and FREDESVINDA G. FUERTES, 90 years old, widow
and a resident of T. Delos Santos, Poblacion West, Science City of Muñoz, Nueva Ecija under
oath, state THAT :

1. We both know personally and intimately know NATIVIDAD GARCIA STA.


MARIA since we are her childhood friend;

2. We are aware that NATIVIDAD GARCIA STA. MARIA celebrates his birthday
every 4th day of February as she was born on February 4, 1916 at Muñoz, Nueva
Ecija thru parents namely FELIX GARCIA and EMILIA DA JOSE, now both
deceased, and NATIVIDAD was married to the deceased EULOGIO STA. MARIA,
and thus she is now 101 years of age;

3. That we are not in any way related to any of the foregoing person within the fourth
civil degree either by consanguinity or affinity, nor do we have any pecuniary interest
in this matter whatsoever;

4. We have personal knowledge of NATIVIDAD GARCIA STA. MARIA’s good moral


character and decent family; and

5. We executed the foregoing to attest to the truthfulness of the allegations herein and
for whatever legal purpose or purposes this may serve, more particularly to prove that
NATIVIDAD GARCIA STA. MARIA is now ONE HUNDRED ONE (101) years
old.

AFFIANT FURTHER SAYETH NAUGHT.

IN WITNESS WHEREOF, we have hereunto set our hands this 21st day of September
2017, here at Science City of Muñoz, Nueva Ecija.
