AFFIDAVIT OF INCOME

I, EMELITA P. LORENZO, of legal age, married, Filipino, and a


resident of Purok 3, Barangay Libas, Roxas City, Capiz, under oath, depose
and say:

1. My husband works as a fisherman and is earning not more than


Eight Thousand (P8,000.00) Pesos in a month and I, his wife, is
a mere housewife and not earning any income;

2. I have two (2) dependent children;

3. My family and I do not possess any income-generating property,


do not own real property, neither do I engage in any endeavor or
business that derives income;

4. In fact, I have never filed any income tax return for the reason
that I do not derive any income and I am only dependent on the
financial support from my husband who is working as a
fisherman;

5. I do not derive any income aside from the financial support


given by my husband;

6. I am executing this Affidavit to attest to the truthfulness of all


the foregoing and for any other legal purpose, specifically in my
application for employment with the Philippine Statistics
Authority (PSA).
