Directions: Rate your Forecourt Personnel objectively an fairly using the
evaluation tool. A rater may give a score with a decimal point (example: 4.25,
3.78). Please do not change the formula.
I. PERSONAL QUALITIES (30 POINTS)
A. PERSONAL GROOMING AND HYGIENE 5
Reports to work in clean and complete set of uniform
Keeps uniform clean and tidy from beginning until end of shift
Keeps hair short, tidy, brushed, and/or ponytailed
Keeps nails short and clean
B. ATTENDANCE AND PUNCTUALITY 20
Models good attendance and punctuality for peers
Reports to work regularly and on time
Uses biometric machine to accurately log daily time in and out
Uses biometric machine to accurately log time spent during break

Notifies and seeks approval from Immediate Superior for planned and/or unforeseen absences and/or tardiness
Is open to new shift assignments and/or work shift extension
C. APPEARANCE AND COMPOSURE 5
Dynamic, vigilant and proactive
Pleasant and with happy disposition
Demonstrates appropriate behavior in different situations

II. PROFESSIONAL QUALITIES AND RESPONSIBILITIES (35 POINTS)

Is aware of duties and responsibilities in the Forecourt
Reports to Station Supervisor any untoward incident in the station
Reports to Station Supervisor Forecourt Operational Items (FOI) for repair and/or replacement

Adheres to the company vision, mission and core values
Follows company and/or station norms, guidelines and policies
Follows guidelines in using Forecourt Operational Items (FOI)

Assists Forecourt Staff or Trainee with challenges and difficulties
Uses appropriate language and gestures in dealing with peers

Performs the complete steps of 6Ps of Forecourt Service at all times
Maintains a record of Zero Misload and Overload Cases in a quarter
Communicates with customers clearly and respectfully
Handles customer complaints discreetly and in a professional manner
C. PROMOTION OF AN ORDERLY AND SAFE FORECOURT ENVIRONMENT 5
Maintains cleanliness and orderliness of the Station including Forecourt, Restroom, Locker Room, Electrical and
Mechanical Room, and Cashier’s Booth at all times
Practices “Clean As You Go (CLAYGO)” Policy at all times
