SPECIAL POWER OF ATTORNEY

KNOW ALL MEN BY THESE PRESENTS:

That I, ______________, American citizen, of legal age, single,


and a resident of _________________________, hereby name,
constitute and appoint _______________, Filipino, of legal age,
married, and a resident of __________________________, to be my
true and lawful attorney-in-fact for me and in my name, place and
stead to do and perform the following special powers,
Since principal ____________ is currently sick, he is unable to
travel and personally run errands for himself and for his transactions;
the special powers granted are:

1) To transact with the United States Embassy for the claim of all
Social Security System benefits, pension, and other sums, of
___________, as well as all other acts necessary to file claims
which may be due to ________________, from the United
States government, and all other United States agencies; To
claim the sums from said agencies;

2) To transact with the Bureau of Internal Revenue, Assessor’s


Office, Registry of Deeds, Land Registration Authority, and
other agencies, regarding the facilitation of transfer of
properties which are given by donation or by will to the
attorney-in-fact by herein principal;

3) To facilitate the US citizenship application for the daughter of


Principal, in the name of __________________, since she is
the biological child of a US citizen;

4) To withdraw from all my bank accounts in my behalf, including


but not limited to BDO passbook account
__________________;

5) To represent, execute, pay the fees required, sign any and all
document(s)and paper(s) as well as claim/receive for and in my
behalf said document(s)from the concerned agencies or offices in the
Philippines, as well as to do any and all acts in order to accomplish the
aforestated acts and deeds.
Hereby giving and granting unto my said attorney-in-fact full powers
and authority to do and perform any and every act and thing
whatsoever requisite, necessary or proper to be done as if personally
present; and hereby ratifying that our said attorney-in-fact shall
lawfully do and cause to be done under and by virtue of these
presents.
