Michelle Artates executed an affidavit stating that: 1) She was authorized by her friend Rina Inojales to obtain copies of Rina's diploma and transcript from PACE Computer College. 2) Rina graduated from PACE Computer College in 2006 with a Computer Secretarial degree. 3) Michelle was keeping some of Rina's school records, including her student ID card, but recently discovered the card was missing. 4) Michelle searched but was unable to find the lost student ID card. She executed the affidavit to attest to these facts.

AFFIDAVIT OF LOSS

That I, MICHELLE ARTATES, of legal age, single, Filipino, and a


resident of Brgy. Gabu-an, Roxas City, Capiz, after having been duly sworn
according to law, depose and say:

1. That I was authorized by my former college classmate, MS. RINA


INOJALES, to facilitate her request for the release of the certified
true copy of her Diploma and her Transcript of Records with PACE
Computer College, Roxas City;

2. That said Rina Inojales was a student of PACE Computer College,


Roxas City, who has taken a Computer Secretarial Course and
graduated on March, 2006;

3. That after graduation, she worked abroad and she was able to leave to
me some of her school records, particularly her identification card
during school time;

4. However, sometime last week, I discovered that her identification


card was no longer in the envelope where I was keeping the same, and
that, I considered the same as inadvertently lost;

5. That diligent efforts were exerted to locate the same, but said efforts
proved futile;

6. That I am executing this Affidavit to attest to the truthfulness of the


foregoing facts stated herein and for any legal purposes it may serve.

AFFIANT FURTHER SAYETH NAUGHT.

IN WITNESS WHEREOF, I have hereunto affixed my signature on this


25th day of June, 2007 at Roxas City, Capiz, Philippines.
