Students Perception Survey on the use of the Module

The perception survey questionnaire was develop to determine the perceptions of
the students regarding the use of the Module. Each question will be rated using 1-4
scales with their corresponding qualitative descriptions.
Directions: For each of the statements below, please indicate your level of agreement
using the given scale. Kindly put a check (√) mark on the box that corresponds to
