AFFIDAVIT OF KINSHIP

I, MARIA ELENA A. PULIDO, of legal age, Filipino citizen, widow and a resident of
San Miguel Street, Barangay Poblacion IV, Tanza, Cavite, Philippines, after having been sworn
to in accordance with law, do hereby depose and state that:

1. I am the lawful daughter of my deceased parents ESCOLASTICO V. ARAYATA


and PASCUALA D. PADUA;

2. Incidental thereto, my parents ESCOLASTICO V. ARAYATA was died on


September 6, 2002 and PASCUALA D. PADUA was died on June 29, 2010;

3. My deceased parents ESCOLASTICO V. ARAYATA and PASCUALA D.


PADUA were under my care and living with me, being the nearest kin to the family
when they were still alive;

4. I am freely executing this affidavit to attest to the truth of the foregoing facts which I
have personal knowledge of and also for the purpose of securing the duly
authenticated Death Certificate with the Philippine Statistics Authority (PSA), and
for whatever legal purpose this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 5th day of February, 2020 in
the Municipality of Tanza, Province of Cavite, Philippines.
