AFFIDAVIT OF NON-RETRACTION OF APPLICATION FOR OPTIONAL


RETIREMENT

I, TSG GILBERT BEHON HEMOR, 826841 (FA), of legal age, Filipino, married, and
presently assigned at ALPHA BATTERY , 6FAB, AAR, PA, stationed at Barangay Kabingi,
Datu Saudi Ampatuan, Maguindanao, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and state that:

1. I am an active member of the Armed Forces of the Philippines and have served
therein for a total of TWENTY ONE (21) YEARS AND SIX (6) MONTHS upon
retirement;

2. I desire to retire (Optional Retirement) from the active military service under the
existing laws effective 07 February 2022;

3. I will not retract my application for Optional Retirement and not work for the
revocation of my retirement order once issued;

4. I am voluntary executing this affidavit in my own free will and to support my


application for optional retirement and for whatever legal purpose it may serve.
