__I messed up and did subtraction instead of addition for some of the problems. For other problems, I

guessed a lot instead of working through the problem and taking my time to be more accurate._____

Circle two activities you will do to improve your understanding of this concept (in addition to
correcting/redoing the missed problems you would like to attempt again):

1. Complete a “look alike” problem (given to you by Miss Smith), showing all of your steps/work.
2. Create the problem/situation and its solution by using a visual representation (drawing, picture of a
hands-on creation, video, etc.).
3. Play a game (on mathgames.com or another approved site) that helps you practice the skill(s)
that you missed. If using a site other than mathgames.com, please write the name of the site.
4. Explore a website (on the Google Classroom site, or Miss Smith’s website) that provides example
problems for you to view and take notes on. Be prepared to discuss your notes with Miss Smith.
5. Watch a video (on the Google Classroom site, or Miss Smith’s website) and practice/take notes as
you complete the practice problems/examples. Be sure to attach all video notes to this form.

• Previous test with corrections in colored pen (not black, gray, etc.).

• Proof of your activities (attached paper copy, screenshot or video sent via Google
Classroom/email, etc.).
