This interview assessment form contains sections for three interviewers to provide ratings and recommendations for a candidate applying for a specific position in a department. The interviewers will rate the candidate on their education, relative experience, job knowledge, attitude, potentials, and personality on a scale of 1 to 5. Each interviewer can then recommend the candidate for hiring, active file, or rejection based on their impressions.
