AFFIDAVIT OF CONSENT

I, OPHELIA MANGABANG VILLANUEVA, of legal age, Filipino, and residing at Purok


I, Bolbok, Batangas City, after having been duly sworn to in accordance with law hereby depose and
say, That:

I am the administrator of a parcel of land covered by Tax Declaration No. 0033-03465 owned
by Spouses Oliver and Evangeline Mangabang. A copy of the document granting said authority is
hereto attached as Annex “A”;

Contiguous and adjoining to the above parcel of land is the land owned by Mr. ROMULO
REPEL RAYOS and on which he has built upon a three-storey commercial building (6 units);

Considering that Mr. Rayos intends to lease out some or all of the commercial stalls/units
therein and has limited parking spaces out front, he has approached me and requested that a portion of
the land I am administering be used for additional parking spaces;

I hereby give my consent to Mr. ROMULO REPEL RAYOS to make use of a THIRTY
SQUARE METER (30sqm) portion of the land covered by Tax Declaration No. 0033-03465 to be
used as additional parking space for his commercial building;

In witness hereof, I hereby affix my name and signature this 17 th day of August, 2020 at
Batangas City, Batangas.
