PN Compounding: Checklist

This document provides a checklist for pharmacy staff to safely prepare parenteral nutrition (PN) formulations. It outlines guidelines for setup and use of an automated compounding device, criteria for manual compounding of PN, and standards for commercial PN products and order review. The purpose is to promote optimal and safe PN practices by verifying ingredients, concentrations, equipment, and procedures to maximize benefits and minimize risks of this complex therapy.

The American Society for Parenteral and Enteral Nutrition (A.S.P.E.N.) champions the best evidence-based practices that
support parenteral nutrition (PN) therapy in varying age populations and disease states. The appropriate use of this complex
therapy aims to maximize clinical benefit while minimizing the potential risks for adverse events.

The purpose of this checklist is to promote safe practices by pharmacy staff to prepare optimal PN formulations.
