REQUIREMENTS FOR CNC APPLICATIONS

1. Transmittal letter addressed to the Regional Director


2. CNC Application Form (EMB)
3. Zoning Certification (Certified True Copy)
4. Proof of Ownership TCT or OCT (attached lease contract, Deed of Sale,
SPA, Consent, Memorandum of Agreement, etc.)
5. Site Development Plan/Vicinity Map
6. Pictures of the site
7. CNC Payment P100.00 In cash or in Manager’s check/cashier’s check
8. Geographical Coordinates (latitude, longitude)
(sample: 7o 35’ 24.5” latitude; 125 o 33’ 31.41” longitude)

For POULTRY (less than 10,000 heads)

Additional Requirements

1. Barangay Resolution
2. Certification from the Supplier stating the exact number of birds to be
supplied.

For GASOLINE STATION: (less than 20,000 liters)

Additional Requirements

1. Barangay Resolution
2. Certification from the supplier the exact capacity of the tanks and
gasoline to be supplied.

Note: ALL DOCUMENTS SHALL BE SUBMITTED IN 2 COPIES TO


BE PLACE IN 2 FOLDERS
