AFFIDAVIT

No_____________________, declare on solemn affirmation that whatever information has been
given is correct to the best of my knowledge.

It is further affirmed that:

a. I have not been dismissed or removed from service;

b. I am not an undischarged insolvent;

c. I have not been disqualified to represent an income tax assessee by a Commissioner of


Income Tax or any authority improved to take disciplinary action against lawyers or
registered accountants;

d. I have not been convicted of any offence connected with any income tax proceeding
under the Income Tax Ordinance, 2001, or the repealed Income Tax Ordinance, 1979
and Income Tax Act 1922; and

e. I have not been convicted of any offence under the Pakistan Penal Code.
