CLIENT INSTRUCTIONS:

1. Prior to every release of loan proceeds, this form should be accomplished by you, your project engineer or any authorized
representative who is familiar with the construction project. Accomplished form should be submitted or faxed tou your account
officer for approval and loan release.
2. Indicated are building compositions for each required percentage completion. These are premised on standard construction
procedures and construction materials installed. Simply check an activity if it is fully completed, partially completed or not yet
started.
3. Any revisions from submitted building plans and bills of materials may alter suggested compositions. Kindly inform your account
officer of these revisions.
4. Partial, Partially installed / completed is equivalent to at least 50% accomplishment.
