SUBJECT: Suspension Letter

TO: Ms. Ayeen Laura de Guzman


Head Nurse – Nephrolife Dialysis Center

This letter is to serve as a Suspension Letter due to your negligence as the Head Nurse of
Nephrolife Dialysis Center Co. secondary to the failure to properly refer the patient to the
Attending Nephrologist (Dra Mara Tugade). In line with the patient’s reaction to blood being
transfused during the Hemodialysis treatment that has resulted to patient being dead. Being
the senior nurse on duty, it is your responsibility to follow up your juniors to make sure that
protocols are being done and followed and assuming things is not included. In addition, being
the Head nurse, it is your job to follow up the patient at home to make sure that the patient is
doing just fine after the incident.
It has been made clear to all that timely referral of patient to the Attending Nephrologist is
expected from all staff. Moreover, your Clinical manager has specifically instructed you to
update Dra Mara about the incident happened to the patient, but there was no referral made
that day. And because of the failure to inform the Nephrologist, the patient was mismanaged,
no medications were given for the blood transfusion reaction.
Your suspension will run effectively on 9/28 until 10/10. Prior to your return, you will secure
your new center of assignment and schedule through Doc Robert and Dra Mara on 10/12/2020
via meeting.
