After enhancing your knowledge on Evidence-based Nursing Practice, what do


you think are the main reasons why research evidences are not put into
practice? What do you think must be done to bridge this gap?

Evidence-based research means that the information you use to make decisions about
patient care is based on sound research, not opinion. This means you must search several
sources (published articles in medical journals or in electronic form) for data, results and
conclusions of valid, reputable studies. There have been articles and reports that I’ve read
that there are substantial improvements in health outcomes for patients who are treated
through evidence-based practices. However, there are many impediments which have
made it difficult to implement EBP in healthcare settings especially for novice medical
practitioners. It is difficult to integrate research results onto the practice and it may take
too long if active measures are not taken to accelerate this process. Transference of
knowledge onto the practice is obviously a complex activity, where the context and the
implementation process must be kept in mind prior to any change in behavior. There are
several barriers against the adoption of research in the practice; these are lack of access to
research, poor understanding of the research process, insufficient knowledge for critical
analysis of research, lack of time, and little support from the organizational structure to
change the practice. On the contrary, facilitators exist like a positive attitude toward
research, level of formation, autonomy, attendance to conferences, cooperation, self-
efficacy, work satisfaction, participation in research activities, and time dedicated to
reading professional journals. In the article entitled Barriers to the Implementation of
Evidence Based Practice, it was said that the reason why it is not feasible to expect nurses
to implement EBP in the clinical setting is the culture of nursing practice that represents a
nurse as a care-giver, but not a researcher. Over the decades, nurses are trained to provide
care to patients. Therefore, emphasis lies on how nurses can provide safe, efficient and
quality care. As such, a nurse is perceived as a care provider, but not necessarily as a
researcher. In reality, a care provider, as it is the case of a contemporary nurse, applies the
available interventions to address health problems. In most cases, nurses the knowledge
and skills they acquire during their education and training in their practice. This implies
that they only apply what they learn at college. As a result, most nurses prefer using single
interventions which they are familiar with. And in addition, according to Tagney and
Haines (2009), there are eight challenges facing implementation of EBP and these are: (1)
lack of knowledge on use of evidence-based practices. It has been shown that most nurses
do not have enough knowledge to integrate research findings in their practices. (2)
Misperception and negative attitudes on research and evidence-based practices. Most
nurses have a negative attitude towards research and do not understand what entails EBP.
(3) lack of knowledge on how to carry out search and appraise best practices; (4) the
overload of nursing work that leaves them with no time to carry out research; (5)
constraints within the organization; (6) contradictory patient’s expectations; (7) the fear
for one to have different practice from what other nurses or medical peers are practicing
which directly emanates from organizational culture; (8) and last factor is large amount of
information that can be found in nursing journals and books which sometimes may be
confusing to practitioners. To bridge the gap from research to clinical practice can be
accomplished in many ways such as providing more nurses in the health care settings so
there will be less work for them, practice and study in a short time to carry out effectively
EBP. And one model for guiding nurses through this process includes these steps: assess
the patient and the need for change, construct a clinical question derived from the patient's
problem, acquire the best evidence through a thorough search, appraise the evidence for
validity and usefulness, return to the patient and integrate that evidence with clinical
expertise and patient preferences, apply what you have learned, and evaluate what you
have learned. (Pipe, 2015).
