UNDERGO COVID-19 NASAL SWAB TEST

Dear Director Magpantay:

We would like to thank you and your staff for the conduct of a thorough contact tracing in our
agency yesterday regarding the recent incident where one of our clients on Wednesday 10
June 2020 later tested positive for COVID19. The said incident caused fear and panic
among our employees, it can be noted that the agency was only informed 5 days after our
client has been confirmed positive for COVID19.

As an agency that caters frontline services, we are greatly aware of the risk that comes in
opening our agency to our clients. While it is our duty to provide essential services to the
public, it is also our obligation to protect our employees from infectious diseases.

In view of this, to ensure the health, safety and welfare of our staff and our clients, may we
humbly request your good office for our officials and employees to undergo COVID-19 nasal
swab test. This swab test can prevent the risk and identify those employees that are non-
