Short Term

The nurse diagnosed the client with ineffective renal perfusion based on elevated blood pressure, low urine output, edema, and abnormal lab results. The short term plan is to monitor intake/output, behaviors, educate the client on their condition and interventions, and decrease edema and labs. The long term goal is normal lab results and health behaviors. Interventions like monitoring intake/output and administering diuretics aim to improve renal function and perfusion by reducing excess fluids and waste. Regular evaluation of labs, intake/output, behaviors, and edema will assess the effectiveness of the plan.
