Assignment on model pharmacy in Bangladesh

Introduction

Model pharmacy is a Community-based pharmacists who responsibilities include: checking and


dispensing of prescription drugs, providing advice on drug selection and usage to doctors and other
health professionals and counseling patients in health promotion, disease prevention and the proper
use of medicines. England has 11,400 community pharmacy.

Today model pharmacists are playing an important role in any country as they take
responsibilities of patients’ medicine related needs to assure a better healthcare. Most model
pharmacists in the country still can hardly offer patient‐oriented service. The role of
pharmacists in the community as well as their medicine management may change in the
pattern of the rapid growth of domestic medicine use and national healthcare expenditure. In
most countries regulatory authorities govern how dispensaries may operate, with specific
requirements for storage conditions, equipment and record keeping. For these purpose
Bangladesh Government would be established this service and the drug control agency of
government is working for stopping fake drug sales in Bangladesh.

History of model pharmacy in Bangladesh

From December 2016, a great number model pharmacies has been established such as Lazzpharma
at Dhanmondi, Biomate at Green Road, AKS pharma at Chankharpool, .Gradually this service has
been reached in all districts that Health Ministry has informed. Model pharmacy is one of the
greatest achievements of present government in Bangladesh. On the other it creates the new
job opportunity for graduate pharmacists and non- graduate pharmacists.
Major General MdMustafizur Rahman, Formar Director General of the Directorate General of
Drug Administration (DGDA), said that they had made a policy for the pilot initiative of creating
model pharmacy.
“We hope to cover at least 150 ‘A grade’ pharmacies and 2000 ‘B grade’ pharmacies by this
year,” he said at a press briefing at the DGDA.
Those pharmacies will sell drugs obeying with all standards comprising appointment of
graduate pharmacists. They will also brief customers about the drugs they sell. It’ll be a public-
private partnership.
“If successful, we’ll spread the model pharmacy across Bangladesh gradually.” He later added.
At last we can say that the unemployment problem will be removed and the service of the
health will also be improved in our country by continuing this project.

An ideal model pharmacy according to DGDA

The new name of service for health is model pharmacy.In this model pharmacy should have A-
grade, B-grade and two C- grade registered pharmacists besides 300squares area and proper
temperature for keeping medicine.The roof of this building should have proper distance and
the temperature of the frizz should be right beside the prescription service desk. On the other
hand this model pharmacy should have separate corner for pharmaceutical products and none
pharmaceutical products and instruments. Model pharmacies will be allowed to stock and sell all
non-prescription(OTC 39) medicine registered by DGDA. It shall be maintained a record of all
medicine dispensed in a register approved by DGDA .For each prescription dispensed a record
shall be made as follows:
1. Serial Number of entry,
2.Date of sale,
3.Name/code number and address of the prescriber,
4.Name of the Patient and condition for which the prescription was written.
5.Name of the drug preparation and the quantity of suppliant.
The model pharmacy should maintain automated system to preserve all suppliers receipts and
invoice for prescription and non-prescription medicines and store all hard copies of the same
supplier invoices and receipts on the premises for non less than two years. A purchase record
book shall be kept, which shall minimally include.
A patient are getting extraordinary service in model pharmacy. An (A- Grade) Pharmacists are
counselling about the fitness and diseases of the patient and he is informing the patient about
the adverse effects, side effects and also tell how to take medicine. This is the first time,this
service has been started in our country.
Mythology and Result
In this survey 20 Pharmacy data has been collected around Dhaka. Survey paper is adorn with
20 ideal question according requirement for accredited model pharmacy by DGDA to take this
data. Data on model pharmacy’s management in Bangladesh. By this survey we can know that
how pharmacy management there dispense drug. Are they obeyed all the rules and regulation
of DGDA. It should dispose in the right ways in our country, because this sector has a great
influence on health sector and patient disease cure.

1.How many pharmaceutical personnel working?


It is matter of hope that currently all of the pharmacy has at least 1 pharmacist in the
management of dispense medicine. Approximately 30% A-grade pharmacist working in those
pharmacy and 35% B-grade pharmacist work in different pharmacies. Though a lot of C-grade
pharmacist work in the pharmacy and all the pharmacy has non pharmaceutical personnel in
the management of medicine dispense.

2.Do the pharmaceutical personnel complete any dispensing course?


This result was very good due to 90% pharmacy has pharmacist so they all know about how to
dispense drug to customer or patient.
3. Is the model pharmacy minimum 300sq feet space with 8 feet height?
For the survey I went all theestablish and well decorate model pharmacy so 95% pharmacy has
enough space for require a model pharmacy under DGDA policy.
4. Is the pharmacy maintain below 30℃ temperature?
All the pharmacy was well air controlled and enough environment for dispense drug. So 100%
pharmacy has fulfil the accredited requirement of the DGDA.
5. Is there any record book for dispensed medicine?
45% pharmacies have record their dispense drug. Rest of pharmacy just record on their storage
and available medicines.
6. Which medical services they provide?
As a model pharmacy most of them provides various medical services by the pharmacist such as
BP checking, sugar level checking, First aid, weight check and Nebulized for asthma patient.
7. Is there any source of pure water for compounding?
Now in Bangladesh model pharmacies are not well established. They only dispense the
medicine as a unit dose which is commercially manufactured by various pharmaceutical
companies. 0% of these pharmacy have this services for instant compounding medicine like first
world counties do as example USA, Canada, UK and also third world country like India.
8. Are they counselling to the patients about drug information?
As a model pharmacy every country have to provide counselling service but due to lack of A-
grade pharmacist only 25% pharmacy provide this services.

9. Is there any non-pharmaceutical product registered by DGDA?


95% of pharmacy have various item to sell beside pharmaceutical product such as baby food,
sanitary napkin and other items.
10. Is there any pharmacy grand refrigerator?
95% of pharmacy have proper storage for freezing product under the policy of DGDA.
11. Is they provide any medicine without prescription?
70% of the model pharmacy dispense medicine without prescription. If those are OTC than it
should be ok but various anti biotic and cancer drug are also dispense without prescription. It
happen due to lack of proper maintained of pharmacy and graduate pharmacist.
12. Are they consult with responding doctor about patient condition?
This service are not running in Bangladesh so 0% of pharmacies consult without doctor.
13. Which type of medicine are mostly sold by them?
Mostly these pharmacies sold open the counter drug more than other drug suchas pain killer,
antacid and antipyretic drug.
14. Is they have any economies factor to sell medicines of a particular pharmaceutical
company?
45% pharmacy said that they have influence to provide a particular pharmaceutical products,
they have get more interest to do this.
15. Do they sell for best quality medicine while purchasing rather than doctors choice?
15% of pharmacies try to do this if the doctor’s recommended product are not available in their
store. However the quality are not the factor in that case.
16. Do they provide any online services?
In Bangladesh model pharmacies are not well establish so 0% of pharmacy give online services.
17. Do they provide direction for using medication on time and proper dose?
100% of pharmacy give this type of instruction to patient and their attendance, mainly
pharmacist do this.
18. Are they aware to patient about antibiotic resistance?
It is a matter of sorrow that in this sector model pharmacies are not much concerndue to lack of
A-grade pharmacist. Only 45% of pharmacy aware patient about antibiotic resistance.

19. Are they remove all the expired and damaged medicine from dispensed sell?
100% pharmacy have proper maintenance about damage and expire date medicine.
20. Do they follow all the rules and regulations of DGDA?
100% of them follow the rule of DGDA.

Conclusion
Model pharmacy is a vital sector for health care in every country. If this sector increase
gradually it has a great impact on medical service in Bangladesh. However in Bangladesh that is
not well established. Some of renounced private hospital provide this royal service for cure
disease with all the rules and regulation under DGDA. But most of the model pharmacy do not
have graduate pharmacy for drug management and dispense service. So in Bangladesh model
pharmacy use as a name only but patient do not get any services as needed. So this is a concern
issue for all and it should be proper maintenance by DGDA.
