AFFIDAVIT OF UNDERTAKING

I, JENIFER LUTO BASILIO, of legal age, single, Filipino citizen and a resident
of Brgy. 1, San Francisco, San Nicolas Ilocos Norte, Philippines under oath states that:

1. I am the applicant for zoning clearance for electrical permit on a portion of


parcel of land designated as lot No. 3636-A situated in Brgy. 16, San Marcos,
San Nicolas, Ilocos Norte;

2. The above-mentioned property is subject to application for locational clearance


of zoning compliance for residential house;

3. In compliance with the requirements for zoning clearance, I undertake to


address any or all possible objections or whatever environmental concerns of
the neighborhood or locality that may be affected in the operation of the
intended residential house, and will institute effective and necessary measures
required by the municipal government in strict observance of pertinent laws,
rules and regulation;

4. In the event that there are legal issues or questions regarding the lot where it
stands and operate, I undertake to abide measures and orders from the
concerned courts/agencies and the municipal government, and waive and
renounce my right for possible stoppage of its operation whenever necessary as
may be consistent with law;

5. I am executing this affidavit to attest to the veracity of the above-mentioned


facts thereby satisfying the requirements of the HLURB relative to the
application for locational clearance/ Certificate of Zoning Compliance for the
abovementioned project and for all legal intents and purposes it may serve.

AFFIANT FURTHER SAYETH NAUGHT.
