Speech Language Evaluation and Report

As part of the funding process, a Speech Language Pathologist (SLP) must evaluate and report on their
client’s medical need for a speech device. These are a few requirements -

Speech Report by an ASHA certified SLP – Our AAC Consultants are available to help answer questions
about report writing but are not able to physically assist with the report itself.
• Client’s Background Information
• Evaluation Date
• Speech and Language Abilities
o Receptive
o Expressive
• Speech/ Communication AND Medical Diagnosis
o For Medicare, the speech diagnosis must be SEVERE
• Cognitive Abilities
• Physical Abilities
• Mobility Status
• Hearing and Vision Status
• Daily Communication Needs
• Rationale for Device Selection
• SGDs Considered and/or Trialed
o It is very important to be sure that your funding source does or does not require a trial
period with the recommended device
o Must include reasoning for why other devices are not appropriate for the specified
patient. This list could include other devices that were trialed as well as other devices
that were considered/discussed but not trialed.
• Recommended SGD and Accessories
o Must include a list of the recommended device and all accessories. These should be listed
individually and not inclusive: i.e. I-12 speech generating device, eye gaze accessory,
folding wheelchair mount. Rationale should also be provided for each item and it’s need.
o Must include rationale for the access method being recommended and reasoning for why
other methods are not appropriate. In general, this list should run least to most
expensive: touch access, scanning with switch, joystick control, headmouse, eyegaze.

o Include Short Term and Long Term Goals
• Non-Conflict Statement and Signature
o Remember to Sign and Date the Report!!
