Filial respect and warmest greetings.

Last May 2019, I was not included in the Pay roll. The possible reason is because I was in
Maternity Leave inclusive January 5 to March 5, 2019. To this regard, I am submitting my
documents to justify my claim to be once again be included in the list of Pay Roll.

Attached hereto are the following documents:


1. Photocopy of April 2019 Pay Slip;
2. Photocopy of Approved Maternity Leave Form;
3. Photocopy of medical Certificate
4. Reinstatement
5. Special order (S.O)
6. Photocopy of Appointment
I hope that this request is justified and my claim to be included in the pay roll again be approved.
