MANIFESTATION

WHEREAS, presented to this Body for appropriate action, is the request of the
Provincial Governor, Honorable Jayvee Tyron L. Uy, MPA for the modification of
the amounts reflected in the ratified Contracts entered into by the undersigned with
the following Contractors;

WHEREAS, upon scrutiny on the Contracts, it was found out that the some of the
figures appearing therein were not the same with the Project Cost as audited by the
Commission on Audit (COA);

WHEREAS, such deviation or variance from the Contract Price is allowed on


account of untoward or extraordinary circumstances and upon prior approval of the
Government Procurement Policy Board (GPPB);

WHEREAS, to avoid variance or area of confusion of the costs of the subject


projects, the undersigned would like to modify the amount of the Contract Price to
the amount of the actual Project Costs, to wit:
