This Survey Questionnaire form is part of my on-going thesis


entitled TEACHING SKILLS AND PERFORMANCE OF TEACHERS IN RELATION
TO ACADEMIC ACHIEVEMENT OF GRADE 9 STUDENTS of Junior High School
in the Division of Bago City. Kindly fill out the data and your

9 Teachers in the Division of Bago City. Below are the
statements, which identify the skills of teachers in terms of
Instructional Skills, Classroom Management, Value Integration,
and Professional Competence. Kindly rate the degree of your
