This Memorandum of Agreement (hereinafter referred to as “Agreement”) is made and entered

into in the City of Manila by and between:

Development, DR. Anna Ruby P. Gapasin, hereinafter referred to as UNIVERSITY;

WITNESSETH THAT:

WHEREAS, the UNIVERSITY has instituted a service project wherein students and faculty
members of the university’s Department of Master in Information Technology. shall develop an (name of
software) for the (name of client’s office) as part of the graduate school’s effort to undertake IT
extension projects and activities that will facilitate the transfer and adoption of appropriate technology

that could contribute to community development, thereby enhancing their social responsibility and

awareness as supervised by its faculty members;

WHEREAS, (name of client’s office), will collaborate by providing all the data and documents
needed for the herein mentioned system development and allow the employees of its department to use
the developed software, has agreed to provide support to the faculty and students of the POLYTECHNIC
UNIVERSITY OF THE PHILIPPINES, Graduate School – Master in Information Technology in their
aforesaid endeavor,

NOW, THEREFORE, for and in consideration of the foregoing premises, (name of Client’s
Office) and UNIVERSITY hereby agree on the following:

A. The ( Name of the Organization ) shall:

1. Provide all the data and documents needed for system development.
2. Provide the hardware and software requirement of the system
3. Provide venue and facilitates user’s training

4. Provide certificate of User’s Acceptance of the said system

5. Provide certificate to the IT volunteers from the Department of Master in Information
Hon. Hermenegildo E. Delatado

6. Claim co-ownership for whatever output/result/technologies generated from the extension
projects that will merit intellectual property application, publications or use, subject to the
policies of the UNIVERSITY relating to copyright and intellectual property rights.

B. The UNIVERSITY (Graduate School MIT) shall:

1. Take charge of designing and developing system for ____________________________;

2. Designate a representative who will coordinate with the Organization on various development
of the system;
3. Provide handouts and conduct training for the employees who will use the developed system;

4. Provide evaluation report i.e. pretest and posttest and certificates to participants (Certificate of
Participation/Certificate of Completion) based on the evaluation report;

C. DURATION
This agreement shall be effective on (date) and for the duration of the School Year
