This is to certify that ANTONIO C. MORENO, Filipino, and a bona fide resident of
barangay Caub, Del Carmen, Surigao del Norte is known to be a good person with good moral
character and a law abiding citizen. In connection with this, the name mentioned above is
known and acknowledged to be a member of indigent for he belongs in the low income family.

This certification is issued upon the request of the interested party for the purpose of
