Exercise 1-Management of Client With Ear Problem Review: Anatomy & Physiology

The document provides an overview of common tests used to evaluate ear problems: 1) An otoscope exam assesses the ear canal, eardrum, and middle ear by gently pulling the ear outward and using a speculum and light to visualize the structures. 2) Audiometric testing determines a patient's hearing levels using tones and speech played into each ear at different volumes and frequencies to create a audiogram graph of results. 3) Tympanometry measures the function of the eardrum and middle ear by placing a device in the ear canal to change pressure and measure vibration responses, represented on a tympanogram graph.

Exercise 1- Management of Client with Ear Problem

Review: Anatomy & Physiology

1. Label the parts below.

Answers:

1. Semicircular Canals
2. Auditory Nerve
3. Eustachean Tube
4. Outer Ear
5. Malleus
6. Stapes
7. Helix

8-20. Describe the purposes and proper procedure for use of the following hearing tests as
shown in the video.

A. Otoscope Examination- The otoscope exam helps to assess the condition of the external
auditory canal (EAC), tympanic membrane (TM), and the middle ear. Mastering the
otoscope exam leads to accurate diagnoses, allowing for targeted treatment and
prevention of complications. The otoscopic exam is performed by gently pulling the
auricle upward and backward. In children, the auricle should be pulled downward and
backward. This process will move the acoustic meatus in line with the canal. Hold the
otoscope like a pen/pencil and use the little finger area as a fulcrum. Based on the video,
we should be sure to select the correct “tip” (speculum) to fit the patient’s ear, never
forget to turn on the light, then hold the back of the ear with one hand and gently
introduce the otoscope into the ear canal. Place eye next to the viewing end of the scope
and continue to move the scope and the ear gently and look until you see the ear drum.
The drum can and will have a variety of appearances.

B. Audiometric Testing- it is important to the success of a hearing conservation program

because it helps determine if occupational hearing loss is being prevented by the noise
control measures. It is a graph that shows the softest sounds a person can hear at different
pitches or frequencies. The closer the marks are to the top of the graph, the softer the
sounds that person can hear. Where the patient's results fall on the audiogram indicate the
different degrees of hearing loss. This procedure involves using an audiometer, which is a
machine that plays sounds via headphones. The audiologist will play a variety of sounds,
such as tones and speech, at different intervals into one ear at a time, to determine
patient’s range of hearing. The audiologist will give instructions for each sound.

C. Tympanometry- tympanometry is a medical test that measures the function and

movement of the eardrum and middle ear. The results of tympanometry are represented
on a graph called a tympanogram. The test is usually quick and painless, unless the
eardrum or middle ear are inflamed. For this test, a device is put inside the ear canal,
changing the pressure and making the eardrum vibrate. The test measures changes in
vibration and records them on a graph.
