NEFT Form

This document is a form for an individual to provide their bank account details to the Life Insurance Corporation of India (LIC) in order to receive policy payments via NEFT electronic funds transfer. The form requests the individual's policy number, name, bank name and branch address, account type (savings, current, etc.), account number, IFSC code, email, and enclosed proof of account ownership (cancelled cheque or bank passbook details). Submitting this completed form will allow the LIC to deposit policy payments directly into the individual's bank account through NEFT and notify them via email.
