Re: Incident Report on the Incorrect/Improper Loading of Fuel into my Unit
I then noticed that instead of Shell V Power Gasoline, diesel was instead pumped into my
gas engine car. I immediately called the attention of said Shell attendant. Ms. Duarna
accidentally pumped Shell V Power Diesel instead of Shell V Power Gasoline.
It’s a good thing that I didn’t start the car. I asked for assistance for the removal of the
contaminated gasoline from her supervisor, Mr. Felipe Suyo, who corrected said mistake by
draining the gas tank, flushing the gas tank and replacing the fuel with the correct Shell V
Power Gasoline. Aside from these, no other steps were taken by the station and its
attendants to check if further damage was caused to my unit due to the said
misfuelling.
I have immediately filed an Onsite Incident Report immediately thereafter (a copy of
which is attached to this letter). I believe that the said misfuelling may cause potential
damage to my unit which may entail additional costs to me in the thousands of pesos. Said
incident is an unwarranted situation that every car owner would want to avoid at all costs.
I want to report this incident to stress that the attendants in this case are at fault for clearly
for the negligent loading the wrong type of fuel into my car, and that said attendant and her
immediate supervisor should be accountable under the circumstances. They should be liable
for damages that my unit would entail caused by said negligence.
I hope that this incident report would merit your attention and prompt response/action.
Thank you.
