SAFETY ASSESSMENT
1. FOUNDATION Y N N/A
Stable ground floor (not wet or slippery)
Timber sills (sole plate-30”x9”x1-1/2”) used for soft surfaces i.e. sand, asphalt, etc.
Steel base plate used as mounting for posts (standards)
If screwjacks are used, maximum thread adjustment not to extend beyond 2/3 of its
length
Scaffold clear of vehicle traffic
2. TUBES
Tubes made of mild steel 48.3mm (1.9”) OD
Completely straight, free of bents, cuts, excessive rust
3. POSTS (STANDARDS)
Posts (standards) plumb/vertical
Joints in adjacent posts (standards) are not in the same lift (staggered joints in
standards)
All joints in posts (standards) connected with a joint pin or sleeve coupler
Span between posts (standards) in both directions not more than 2 m (6.5 feet)
For a system scaffold, all joints in posts or standards connected using a spigot
4. SCAFFOLD LIFTS
Runners (ledgers) and bearers (transoms) horizontal
Runners (ledger) have staggered joints i.e. joints in adjacent ledgers are not in the
same bay
Runners (ledgers) interlocked to the inside and the outside of posts (standards) at
even heights
Vertical spacing of runners and bearers not exceeding 2m (6’-6”)
Bearers and board members setting on top of runners (ledgers)
Scaffold base lift installed about 6” above the scaffold base
5. TIES AND BRACING
Adequately tied to the building or structure
Tied at every 25 ft. intervals, vertically and horizontally
Free-standing tower scaffolds guyed/tied adequately
Vertical diagonal bracing provided (in both directions) for scaffold’s full length
In tower scaffold, plan (horizontal) bracing provided at the base, top and at every
3rd lift
6. DECKING
Planks free of cracks, defects, warping
Minimum dimension of planks is 2” x 9”
Planks extended over the support by not more than 6”
Decks closely planked
Free of unnecessary obstructions, tripping hazards
Each plank on 3 supports (bearers not more than 1.5m (5 feet) apart
Planks secured
Platform has adequate room for men and materials

7. GUARDRAIL SYSTEM
Toprails, midrails, and toe boards provided on all open sides and ends
Toprails leveled between 38” and 45” above working platform
Midrails leveled halfway between toprail and working platform
Guardrails securely fixed to the inside of standards
Toe boards securely fixed to the inside
Toe board is 6” in height
8. ACCESS TO THE PLATFORM
Safe access provided i.e. ladder, stairways, ramps, etc.
Ladder free of obvious defects such as missing rungs
Ladder extends 36” above landing platform (or 3 rungs)
Ladder firmly secured
Vertically positioned straight ladders secured at the top, middle, and bottom
sections
For non-vertical positioned ladders, slope ratio of 4:1 observed
Safe landings provided at every 30’ (9 meters)
Access holes through platforms adequately protected
Ladder placed at every 35’ span
9. GENERAL REQUIREMENT
Scaffold design and erection meet the minimum standard requirements
Location at a safe distance from overhead power lines
Location at a safe distance from excavations
Non-conductive ladders not used near electrical equipment
Aluminum ladders not used near contact with caustics and acids
Personnel who are likely to fall from a height of at least 6 feet (1.8) provided with full
body harness
10. MOBILE AND TOWER SCAFFOLDS
Mobile and tower scaffolds horizontally and vertically braced
Plan bracing installed at the base, top, and at least on every 3rd lift
Height is limited to 3 ½ times the shortest base dimension
Shortest base dimension is limited only to 4’’
Diameter of wheels/casters not less than 5”
All casters (wheels) fitted with brakes or wheel lock
Wheel locks use at all times except when scaffold is moved
Maximum height does not exceed 40’’
Structure not moved when workers are on platform
