This is to appoint the designation of MARK EDSON V. ABOY as SBM Coordinator of Waterfall
Elementary School effective June 3, 2019 up to present.

As designated SBM Coordinator you are expected to perform the following duties and
responsibilities:

1. To coordinate with the members of the school assessment team regarding the assessment of
SBM level practice.
2. To conduct meetings relevant to school-based management.
3. To remind faculty members and assigned committees on the submission of various
documents for pertinent school reports.
4. To assist in the safekeeping and organization of school files.
5. To submit an action plan and quarterly accomplishment report on time or anytime as needed
arise.
6. To perform other duties and accepts responsibilities as assigned by the school head.

As you have been appointed, you are obliged to perform the stated duties and responsibilities and
carry out your appointment as your plus factor. However, failure to comply, the undersign has right to
revoke the benefit of this appointment and to determine appropriate action.
