1) That because of the current Covid-19 situation our child has suffered enough academic loss on
account of Educational Institutions been closed.
2) That we do not have enough infrastructure and facility to get the proper online education for our child;

3) Now since government have allowed the children to voluntarily opt to have physical presence in
educational institutions, we would like to opt for said option;
4) We hereby solemnly state and undertake that:

(a) My child will be in the residential facility as will be provided by the educational institution;

(b) My child will abide by the rules and regulations as framed by the educational institution;

5) We hereby affirm that the said option of physical presence of the student in educational
institutions has been opted by us voluntarily keeping in view the future academic prospects of our
child;
6) That during the stay of the child in the residential facility if he/she is sent home owing to medical
reasons, then the educational institutions holds no responsibility.
7) We hereby declare that we have not been forced to make the said declaration and further affirm that
the above undertaking is voluntary and with our free will and consent. In case this declaration is found
to be false/untrue, the admission of our child is liable to be cancelled.
