AFFIDAVIT OF CORRECTION OF ENTRY IN THE CERTIFICATE OF BIRTH

I, LENY VALLEJO GONZALES, of legal age, married, residing at Purok Sampaguita


St., Rio Chico General Tinio, after being sworn to in accordance with law, depose and say:

1. That I am the mother of Sophia Vallejo Gonzales who was born on 16 Aug 1994, a
copy of her certificate of live birth is hereto attached as Annex “A”;

2. That an information in her Certificate of Live Birth has a clerical error;

3. That on section twenty (20) of the said Certificate of Live Birth, under
INFORMANT, I signed my name thereto, but under which, the relationship between
me and my daughter was indicated that I am her “FATHER”, when clearly I am her
mother;

4. I attest to the truth of the foregoing statements to the best of my own personal
knowledge and based on authentic documents or records. I am executing this
Affidavit of Correction of Entry of my daughter’s Certificate of Live Birth to rectify
the clerical error under Section 20 thereof, and to change the relationship indicated
therein from “FATHER” to “MOTHER”.

IN WITNESS WHEREOF, I have hereunto set my hand this 8th day of January, 2021 in
the Municipality of General Tiñio, Nueva Ecija Philippines.
