CONSENT FORM FOR INTERVIEW

I agree to participate to the research study entitled "Daily Struggle of Filipino Jeepney
Drivers: The consequence of COVID-19 in Calamba City, Laguna" supervised by Prof.
Winnie Altuna, conducted by Psychology majors from Pamantasan ng Cabuyao. This study will
be administered by Ma. Angelica Blessing Agaid, Paula Marie D. Capili, Jessa Mae M. Dela Cruz,
Ellen Jane J. Hacildo, Patricia Anne P. Moro, and Tiffany Anne O. Santiago.

The purpose of this document is to specify the terms of participation in the study through
interview sessions.

1. I have been given sufficient information about this research study. The researchers has been
explained to me and is clear.

2. My participation as an interviewee in this study is voluntary.

3. I allow the researchers to take written notes during interview. I also may allow the recording
(by audio/video) of the interview. It is clear to me that in case I do not want the interview to be
recorded in video I am at any point of time to withdraw from participation.

4. I have the right not to answer any of the questions if I feel uncomfortable in any way during
the interview session, I have the right to withdraw from the interview.

5. I have been given the explicit guarantees that, if I wish so, the researchers will not identify me
by name in any reports using information obtained from this interview, and that my
confidentiality as a participant in this study will remain secure.

Sumasang-ayon ako na maging kalahok ng pananaliksik na pinamagatang “Daily Struggle of


Filipino Jeepney Drivers: The consequences of COVID-19 in Calamba City, Laguna"
sa ilalim ng pagpapayo ni Prof. Winnie Altuna, isinagawa ng mga Psychology majors mula sa
Pamantasan ng Cabuyao.

Ang pag-aaral na ito ay pinangunahan nina: Ma. Angelica Blessing Agaid, Paula Marie D. Capili,
Jessa Mae M. Dela Cruz, Ellen Jane L. Hacildo, Patricia Anne P. Moro at Tiffany Anne O.
Santiago.

Ang sumusunod ay ang layunin ng dokumentong ito, inilalahad ang aking mga tuntunin hinggil
sa paglahok sa naturang pananaliksik sa pamamagitan ng pakikipag panayam/interbyu:

1. Naipaalam na sa akin ng lubos ang lahat ng impormasyon hinggil sa pag-aaral na ito. Malinaw
at detalyadong isinaad ng mga tagapanaliksik bago ang panayam.

2. Ako’y boluntaryong naki-isa bilang kakapanayamin sa nasabing pag-aaral.

3. May pahintulot ang mga taga-panaliksik na magtala sa oras ng panayam. Gayundin ang
pagkuha ng audyo at bidyo sa panayam. Malinaw sa akin na maaari kong bawiin ang pahintulot
sa anumang oras ko ito naisin.

4. Sa panayam, may karapatan akong hindi sumagot sa mga tanong na hindi ako kumportable.

5. Pananagutan ng mga taga-panaliksik ang anumang sensitibong impormasyon tungkol sa akin


o ang seguridad nito. At kung aking nanaisin, maaaring manatiling kumpidensyal ang aking
pagkakakilanlan na naitala sa panayam.
