Request For Datafix BIR

I, Catherine F. Dalleon of RC PAWNSHOP, located at Candelaria St., Brgy. Poblacion Norte,


Paracale, Camarines Norte with TIN No. 925-525-605-000, requesting to your office for a Data Fixing for
the January 2018 return .

I received a letter from your office this month that from your records show that I filed a Monthly
Percentage Tax Return for the period od January 31, 2018 on February 5, 2018 amounting to Php.
3,357.27 but failed to pay the same. I would like to inform that said return was already paid on April 24,
2018 as per Quarterly Percentage Taxable Amount is Php 118,593.50 and the Tax Due is Php. 5,929.67.
with ROR No. 109897.

Attached herewith is a photocopy of the said return.
