SACPMPC SAFETY REPORT SUMMARY 

1. PROCUREMENT MANAGEMENT  
● Is there a procurement plan in place?
● Does the SCM plan provide HSE Requirements?
● If you had subcontractors, did you assess them as per the plan and including HSE
Requirements?
● How was the plan implemented?
● Any HSE Risks identified and controlled during procurement processes.
● Any indicators/ objectives procured during the project.
● Key HSE procured items.

2. COST MANAGEMENT  
● Any HSE costing system.
● Was the BoQ considered in terms of costing?
● Do you have Monthly expenditure for HSE items (PPE, Signs etc.).
● Any failure costs identified during the project (Incidents costs, maintenance).
● System for incident costing for both insured and non-insured.

3. HAZARD IDENTIFICATION  
● Is there a detailed procedure for HIRA?
● Has it identified all HSE hazards as per the process plan?
● How often was the risk register updated?
● Any training provided on HIRA.
● Any HSE appointments related to HIRA.

4. RISK MANAGEMENT  
● Any risk analysis methodology used.
● Process for risk management/ control (considering the hierarchy of controls)
● Highest risk/ impacts identified and how were they managed?
● Are critical task having WSWP or Method Statements (Excavation, working at heights,
confined space etc.).

5. ACCIDENT MANAGEMENT  
● Any incident reporting system/procedure?
● Any incident classification criteria (external and internal incidents)?
● What’s that the project overall DIFR score?
● Highest injuries / damages incurred?
● Incident investigators trained/ approved?
● Any near miss management process?
● Is there any incident investigation toolkit.

6. LEGISLATION 
● Does the company have a project HSE legal requirement/register?
● Any HSE legal compliance audit conducted monthly?
● Was any training done related to Legal Liability?
● Legal updates based on new laws done and which are those laws.

7. HEALTH, HYGIENE AND ENVIRONEMTAL MANAGEMENT 


● Health stressors identified and controlled.
● Action plan progress relating to hygiene survey.
● Waste management program in phase.
● Highest stressor identified.
● Any environmental impact identified and controlled.

8. COMMUNICATION MANAGEMENT  
● Any communication register/ plan
● What communication style/ method used.
● Any communication process to external parties.
● How was language barrier sorted out?
● Toolbox talks development process,
● Awareness needs analysis?
● Training Plan?

9. EMERGENCY PREPAREDNESS MANAGEMENT 


● Plans related to emergency.
● Emergency scenarios identified
● Any drills conducted.
● Emergency contacts in place.
