Dear Sir,

I would like to request to your good office to secure and provide a copy of the
following documents :

1. Approved Plans ( Architectural, Structural and Sanitary-Plumbing)


2. Approved Sanitary Permit

For our project Hallare Building located at 10 East Capitol Drive Brgy.
Kapitolyo, Pasig City. I would like to inform that the said documents were lost due
to the fault of our past contractors for not securing an own copy and it was also said
that it was already submitted to your office long before the new administration. We
already requested for a copy but the only the following documents were retrieved or
given to us were approved ancillary permits (Building permit, Civil Structural
permit, locational Clearance and Fire safety evaluation certificate).

I would be highly obliged if a new copy can be provided to us so that we may be


able to complete our requirements for our on-going application for occupancy permit
and others.
