The _______ National High School is an institution in this Division that implement the K to 12
curriculum with both Junior and Senior High School program. Currently, we are offering the Technical
Vocational and Livelihood (TVL) track specializing the Shielded Metal Arc Welding (NC I), and Agricultural
Crops Production (NC I) to our Senior High School students.

We foresee that the enrolment for the coming years particularly the Senior High School will
increase progressively. We received feedbacks from our stakeholders the need to offer another track or
specialization for the Senior HS students. We also understand that our mission is to cater the needs of
our stakeholders and community through quality, equitable, culture-based, and complete education.

Thus, we would like to request the approval from your office to allow us to offer to our Senior
High School students starting School Year 2017-2018 the Academic Track with the General Academic
Strand (GAS).

We are looking forward for your positive response regarding this proposal.
