AFFIDAVIT
I, FRANZ GIUSEPPE CORTEZ, of legal age, Filipino and a resident
of Block 3 Lot 1 Euro St., West Bay Homes Daang Bukid, Bacoor City,
Cavite after having been sworn to in accordance with the law hereby depose
and state the following:
1. I am the owner of one business establishment operating under the name
MUG Pharmacy (The Generics Pharmacy) located at Lot 1 Block 4 BF
Resort Drive Cor. Abel Nosce BFRV Brgy. Talon 2, Las Pinas City
Area Code 1747;

2. That the aforementioned business establishment is engaged in the retail


of pharmaceutical products to end users for personal use;

3. That said business establishment is regularly employing not more than


ten workers specifically three (3) workers all year round;

4. That I execute this affidavit as one of the compliances required to be


exempted from payment of the regular minimum wage amounting to
five hundred pesos daily;

5. That I understand that should I employ more than ten workers or any
misrepresentation on that matter is a ground for my disqualification
from the exemption granted by Department of Labor and Employment
(DOLE);

6. That I execute this Affidavit to attest to the truthfulness of the


foregoing facts.
