AFFIDAVIT OF TAX EXEMPTION

We SPS. ROBERT G. VILLA and ELISA C. VILLA, both of legal age,


Filipinos, with residence address at Natividad Ext. St., Pangil, Laguna, after
having been sworn in accordance with law, hereby depose and state:

1. That we are the parents of ELOISA MARIE G. VILLA,


an incoming Second Year College Student at the Southern Luzon
State University, Lucban, Quezon;

2. That we have no fixed income – I, Robert G. Villa, am


just a carpenter and I, Elisa C. Villa, is a plain housewife and our
income is just enough/ sufficient for our daily needs;

3. That since we are not gainfully employed, we have no


taxable income, and exempted from payment of income tax with the
Bureau of Internal Revenue (BIR); and

4. That we are executing this Affidavit to attest to the truth


of the foregoing facts mentioned above and in connection with the
enrollment of our daughter, ELOISA MARIE G. VILLA.
