DEED OF SELF-ADJUDICATION BY SOLE HEIR

KNOW ALL MEN BY THESE PRESENTS:

That I, PRINCESS ESTER CAMPOS-BARREDO, of legal age, Filipino


citizen, married to JOAQUIN M. BARREDO III, and a resident of 165 G. Reyes
St., San Juan, Metro-Manila, Philippines,

WITNESSETH, THAT :

1. I am the child and sole surviving heir of the late Ester L. Campos who died at
UERMMC, Aurora Boulevard, Quezon City, Philippines on September 5, 2005,
respectively;

2. Said deceased died intestate, without leaving a will and whom, at the time of
her death did not leave any outstanding obligations whatsoever;

3. At the time of her death, the said deceased left certain benefits and
emoluments which are due or payable to her from the Philippine Health Insurance
Corporation (PHILHEALTH);

4. In my capacity as sole heir of the deceased Ester L. Campos, and in


accordance with Section 1, Rule 74 of the Rules of Court, I, PRINCESS ESTER
CAMPOS-BARREDO, do hereby adjudicate unto myself the foregoing
PHILHEALTH benefits.

Department of Foreign Affairs, Manila on July 06, 2001, known to me and to me
known to be the same person who executed the foregoing instrument and
acknowledged to me that the same is her free and voluntary act and deed.

This  instrument refers to a Deed of Self-Adjudication By Sole Heir, pertaining


to the intestate estate of the late Ester L. Campos, consisting of two (2) pages, duly
signed and her witnesses on each and every page, and sealed with my notarial seal.
