This risk assessment form summarizes potential hazards for a manual excavation activity. It identifies 3 main risks: 1) fall into the excavated pit, which could result in serious injury or death; 2) damage to underground pipelines or cables, which could result in injury, loss of production, or serious injury; and 3) collapse of adjoining structures, which could result in injury or death. Current control measures to mitigate these risks include barricading the work area, only allowing authorized personnel, following a permit to work system, proper sloping and shoring of excavations.
