Situation Hello, I have Josephine Morrow an obese, 80


y/o F, who is experiencing a venous stasis ulcer
Patient’s name, age, specific reason for on her right lower extremity.

Background Pt was admitted on Dec. 3rd , 2020


Pt’s main diagnosis is right lower extremity
Patient’s primary diagnosis, date of venous stasis ulcer. Meanwhile her other
admission, current orders for patient diagnosis is chronic venous insufficiency, deep
vein thrombosis (DVT), peripheral artery
disease (PAD), and chronic obstructive
pulmonary disease (COPD).
The current orders read as following:
 Multivitamin 1 tab PO daily
 Zinc supplement tab PO daily
 Aspirin 81 mg PO daily
 Albuterol/Ipratropium inhaler (120/20
mcg/puff) 2 puffs for wheezing
 Tylenol 650mg 2 tab PO q6h PRN for
pain
 CBC and CMP monthly
Assessment Pt is A&O x3
Vitals signs: 120/80, 88 HR, 20 resps, 94%
Current pertinent assessment data using SpO2, 98.6°F
head to toe approach, pertinent
diagnostics, vital signs Pt is on a normal diet and taking nutritional
supplements. Pt needs assistance getting in and
out of the bed. As well as assistance into a chair
and ambulating. Pt is at moderate risk for ulcers
scoring a 14 on the Braden scale. Brown
pigmentation noted bilaterally on lower
extremities with +2 pedal edema
Recommendation I would recommend promotion of circulation
and maintaining a good skin integrity. Avoid
Any orders or recommendations you standing for a long period of time. Avoid sitting
may have for this patient with legs in dependent position (legs dangling).
Maintain an adequate exercise regimen to
promote weight loss. Follow the diet plan with
nutritional supplements. Incorporate
compression stocks to promote circulation.
Assessment of skin should be done daily.
