The undersigned is a Notary Public for Mandaue City under Commission No. ______
valid until __________.

The undersigned had been an in-house lawyer of a private company in Mandaue City
since 2017 until December 2021. Because of the delays in the execution of some
contracts from the counter-parties and the concomitant resignation of the undersigned
from the said company, notarial reports and duplicates of acknowledged instruments
were belatedly submitted. Adding to it the hardships and obstacles brought by quarantine
restrictions due to COVID-19 pandemic, the reports were only now submitted.

The undersigned now begs the Honorable Judge’s indulgence in the herein delay. The
undersigned undertakes to exert more effort and diligence in order that this predicament
will not happen again.
