AFFIDAVIT OF VERACITY / AUTHENTICITY


I, Marvin C. Casalan, of legal age, Filipino citizen and resident of Rombang, Belison,
Antique, after having been duly sworn to in accordance with law hereby depose and state
THAT:

1. I was approved as a CHED scholar under the Scholarships for Graduate Studies-
Local of the K to 12 Transition Program.

2. I am submitting the following documents for my claims of living and other


allowances:

4. I understand that the submission/s shall be subject to due process and may be
approved or disapproved based on the deliberation of the Commission, based on
the policies of the scholarship.

5. I understand that the documents submitted, and related transactions may be


subject to further validation, when able or deemed as necessary.
6. I understand that the Commission reserves the right to hold the processing of
documents which may have been suspected to have been altered, modified, or
similar.

7. I understand that any act of immorality, dishonesty, misconduct, or any acts


which are inimical to the interest of the Commission may be grounds for and may
result in the termination of my scholarship, which shall cause the repayment of all
enjoyed financial privileges.

8. This affidavit is executed to attest to the veracity of the above and in support of
my withdrawal from the scholarship grant.

9. I fully understand the consequences of not giving correct and truthful data and I
am aware that non-disclosure of material information as required will put me
under the pain of perjury by executing this document.
