Order Under The Epidemic Diseases Act, 1897

By virtue of power vested in me under section-2(ii) as 'Authorized Person' vide above said
notification, and section 11 (vii) read with section l2, I hereby declare “The Protocol for Admission of
COVID-19 Patients in Hospitals or under Home Isolation” in Vadodara Municipal Corporation and
Vadodara District ” as per Annexure-1.

VMC and District administration has reviewed the current criteria of admission, verified the same with
various clinical experts and has decided to Notify the Protocol for Admission of COVID-19 Patients in
hospitals or under home Isolation. The Hospitals designated for treatment of COVID-19 from time to time
by VMC will be authorized to follow the above protocol notified herein. This will help reduce the bed
occupancy and reduce the out of pocket expenditure of citizens.

The protocol will be reviewed with progression of disease and be modified depending on the case
treatment and management reviews. Medical Officer of Health in VMC area and Chief District Health
Officer in Vadodara District will arrange for a review of the Admission Protocol in Annexure-1 on a
periodic basis.

This order will be effective from 1st of April 2021 and till further notice by the undersigned.

Any Medical Professional / Hospital / Organization in Vadodara, following the above Admission
Protocol will be deemed to have carried out the treatment in good faith and shall stand protected under the
Epidemic Diseases Act, 1897 vide Notification No. GP/9/NCV/102020/SF-1/G dated 13th March 2020 and
Notification No. GP/9/NCV/102020/SF-01/G dated 23rd March 2021 issued by the Health and Family
Welfare Department, Government of Gujarat

ANNEXURE 1: PROTOCOL FOR ADMISSION OF COVID-19 PATIENTS

CRITERIA FOR HOSPITAL ADMISSION


AGE CRITERIA:
1. Age more than Sixty (> 60) years.
2. Age less than Sixty (< 60) years with comorbidity.
a. Diabetes b. Ischemic Heart diseases
c. Morbid Obesity d. Asthma
e. Renal or Liver Failure f. Hypertension
g. Immunocompromised states like HIV/AIDS; Patient on steroids, malignancies.
CLINICAL CRITERIA:
1. SpO2 (Oxygen Saturation) – Less than (<) 94%
a. Moderate disease – SpO2 88 % to 94 %
b. Severe Disease – Less than or = ( 88 %)
2. Breathlessness
3. Continuous Fever for > 3 days
4. Respiratory Rate – More than 20 per minute
5. Hypotension - SYSTOLIC Blood Pressure: <100 mm Hg
6. Tachycardia – Resting Heart Rate > 100
7. Bradycardia – Resting Heart Rate <50
8. 6 minute walk test induced desaturation (SpO2 < 94%)
LABORATORY CRITERIA:
1. Neutrophil : Lymphocytes Ratio - More than 3.5
2. ABG: PaO2/FiO2 Ratio (P: F): < 300
3. C-Reactive Protein (CRP): More than (>) 25
4. D-Dimer: More than (>) 1000
5. Serum Ferritin: More than (>) 500
RADIOLOGY CRITERIA:
1. X-Ray Chest / HRCT Chest – More than (>) 20% Lung Parenchyma Involved
2. HRCT Chest – CORADS (CT) Score of More than (>) 10/25.

TREATMENT OF PATIENTS IN HOSPITAL


1. Injection Remdesivir
a. 100 gm IV in pint stat and after 12 hours on day 1 (Two doses on day 1)
b. 100 gm IV O.D. for 4 days
2. Injection Dexamethasone 6mg/8mg IV O.D. for 5 days
3. Injection Cefoperazone + Salbactum IV 12 hourly OR Drugs to be used at the discretion
4. Injection Piperacillin + Tazobactum IV 6 hourly OR of Physician/ Consultant
5. Injection Meropenem IV 8 hourly
6. Injection Targocid
7. Injection Tocilizumab IV in pint if oxygen requirement Increases Suddenly
8. Injection Clexane IV OD, Pantocid IV OD
9. Tab. Vitamin C, Zinc, Ivermectin, Torvasan, Mucomix, Sorbex, Montelukast
10. Oxygen Therapy as per requirement; BiPAP/Invasive Ventilator as per requirement
NOTE:
 The Drugs mentioned here are suggestive, Physician/Consultant/Specialist can
provide treatment as per clinical condition of the patient.
 Injectable Drugs may be given post discharge at home under supervision and
consultation of Physician/Consultant/Specialist of the Designated Hospital.

CRITERIA FOR HOME ISOLATION
AGE CRITERIA:

1. Age less than Sixty (< 60 ) years without Co-morbidity

CLINICAL CRITERIA:
1. SpO2 (Oxygen Saturation) – More than (>) 94%
2. Mild Symptomatic
a. Mild Fever for less than 3 days
b. URTI (Upper Respiratory Tract Infection)
LABORATORY CRITERIA:
1. C-Reactive Protein (CRP): Less than (<) 25
2. D-Dimer: Less than (<) 1000
3. Serum Ferritin: Less than (<) 500
RADIOLOGY CRITERIA:
1. X-Ray Chest / HRCT Chest – Less than (<) 20% Lung Parenchyma Involved
2. HRCT Chest – CORADS (CT) Score of Less than (<) 10/25.

TREATMENT OF PATIENTS UNDER HOME ISOLATION


1. Tablet Hydroxychloroquine
a. 400 mg b.d. on day 1
b. 200 mg b.d. for 4 days
2. Tablet Doxycycline (100 mg) 1 b.d for 5 days
3. Tablet Dexamethasone (6mg / 8 mg) 1 o.d. for 5 days (In NON-DIABETIC Patients)
4. Tablet Ivermectin (12 mg) 1 o.d. for 3 days
5. Tablet Zinc (50 mcg) 1 o.d. for 5 days
6. Tablet Ecosprin (150 mg) 1 o.d. for 5 days
7. Tablet Vitamin C 1 b.d. for 10 days
8. Tablet Vitamin D3 (60,000 units) 1 o.d. for 3 days
9. Tablet Multivitamin 1 o.d. for 10 days
10. Tablet Mucinac (600 mg) 1 t.d.s. for 5 days
11. Tablet Montelukast (10 mg)1 t.d.s. for 10 days Drugs to be used at the discretion of
Medical Officer/Physician/ Consultant
12. Tablet Omeprazole or Rabeprazole for 5 days

NOTE:
 All the hospitals/consultants will adhere to the admission criteria and clinical
protocols defined herewith.
 Any person / Institution / organization found violating any provision of these
regulations shall be deemed to have committed punishable offence under section
188 of Indian Penal Code (45 of 1860).

GUIDELINES FOR HOME BASED COVID CARE


1. All Hospitals will provide list with contact number of patients who are home-isolated to Medical
Officer of their respective Urban Primary Health Centre (UPHC) (or UPHC of the area where
the patient resides). Contact details of the Medical Officers are as per Annexure 2.
2. Patient will be followed up by our team of “SANJEEVANI ABHIYAAN” at their home on a
daily basis as per protocol and treated.
3. Patient will have the option of getting teleconsultation from the Hospital or any other Consultant
as per slot available on the ‘TELE-CONSULTING’ Application.
