Nowadays, there are a large number of indictment about medical error in patients being
reported.

The number of patients safety incidents is increasing

World: In USA, Denmark, and Australia there are still 3,2-16,6 % of adverse event and
also medical error occurrence about 8-12% in inpatient room. (WHO, 2014)

Indonesia: There are 114 cases of patient safety incidents in 2009, 103 cases in 2010,
and 34 cases in 2011. (KKPRS, 2011)

West Java: The incidence of patient safety in 2017 was 2,8%. (KKPRS, 2017)

The research of Mutiara et al., (2018) found that clinical education participants are
considered unable to apply a comprehensive thinking system in the implementation of
patient safety.

Cimahi and also the highest level hospital in Kodam III / Siliwangi founded in 1887.

The center of practice land for several higher education institutions in Cimahi

There are 23 educational institutions that have collaborated

This causes a big number of nursing student as participants of clinical education at this
hospital
