Black Belt Project 

Lean Sigma Corporation is the official certification entity for your Lean Six Sigma
Certification. In addition to passing your phase tests and your certification exam, your
Black Belt certification requires the completion of a Black Belt project. Lean Sigma
Corporation requires all candidates to submit a final project report for review and
approval by a Master Black Belt. Once your report has been accepted, you may be
invited to a tollgate by the MBB to present and defend your project work or you may be
asked to make changes, additions and/or revisions for re-submission. 
Report submission is facilitated within this learning management system. As a Black
Belt candidate, you are required to complete this project submission as a part of your
certification. Below are the components and sub-topics that should be covered by your
project report. This represents a comprehensive review of the tools, methodology,
learning and conclusions that should have been recorded throughout your DMAIC
project. There are links down below for a couple of project examples (see attached) as
well as report submission guidelines (see attached). 
Executive Summary 
 Execution Summary Section – the Executive Summary section must provide a
single page summary of the project, problem statement & objective, primary
metric with a “before and after” view of the primary metric. 
 The executive summary should touch on the root cause or critical factors that
were managed to make the improvement with some discussion of the benefit of
the project. 
Define Phase 
 Define Phase Section – The Define Phase section must include the following: 
 Business Case – Why is this project important to the business 
 Problem Statement, Objective/Goal Statement 
 Primary Metric (expressed as an equation and displayed with a time series run
chart) 
 Secondary Metric (expressed as an equation and displayed with a time series
run chart) 
 Cost of Poor Quality 
 Project Scope, Project Plan, Project Timeline & Project Team 
 High Level Process Map 
 Pareto Chart 
Measure Phase 
 Measure Phase Section – The Measure Phase section must include the
following: 
 Detailed Process Map or SIPOC or Value Stream Map 
 Cause & Effect Diagram 
 Cause & Effect Matrix 
 Failure Modes & Effects Analysis 
 Graphical Analysis 
 Attribute or Variable Gage R&R 
 Process Capability 
 List of Possible X’s to be verified or eliminated in Analyze 
Analyze Phase 
 Analyze Phase Section – The Analyze Phase section must include the
following: 
 Discussion of Sampling Strategies Used for Analysis 
 Normality Test 
 Test of Equal Variances 
 2 Parametric Hypothesis Tests (e.g. 2 Sample t, One Way ANOVA etc.) 
 2 Non Parametric Hypothesis Tests (2 proportions, Moods Median etc.) 
 At least one (1) verified critical X that will be tested and deployed in the Improve
phase 
Improve Phase 
 Improve Phase Section – The Improve Phase section must include the
following: 
 Correlation Analysis 
 Simple Linear or Multiple Linear Regression 
 DOE Plan or Study Plan Intended to Prove X influence on Y in a Live
Environment.
(timeline, resource requirement, test variables, measurement system, costs etc.) 
 DOE Results Analysis or Planned Study Results 
 Executive Summary of Improvement Results 
Control Phase 
 Control Phase Section – The Control Phase section must include the following: 
 Control Plan Including Response Plan 
 Communication Plan, Training Plan or Audit Plan 
 Use of 1 Lean Control Method (5S, Kanban, Poka Yoke) 
 Use of 1 Statistical Process Control Chart

When your project report is ready for submission please use the following guidelines 
1. Save your report as a pdf file 
2. Upload your report on this page using the upload feature below 
3. Send an email to [email protected] indicating that your final Black Belt project
report is ready for review
– An MBB may request revisions and/or follow up questions
– If an MBB reviewer approves your project, this requirement will be marked
complete by Him/Her

continue/based from if you can.
